Zirconia vs. Titanium Dental Implants

Titanium Implants

Titanium implants have been used in implant dentistry for decades and remain the most widely studied implant material. They are strong, biocompatible, and highly predictable. Titanium implants integrate directly with the jawbone through a process called osseointegration, creating a stable foundation for crowns, bridges, and full-arch restorations.

Long-term studies show that titanium dental implants have excellent survival rates, often reported around 95–97% in many clinical studies, depending on the patient’s health, surgical site, maintenance, and follow-up period. One review reported a 10-year implant survival estimate of 96.4%, while another review of titanium implants reported an average survival rate of 96.8%.

Titanium may be preferred when maximum strength, long-term data, flexibility in restorative design, or treatment of complex cases is needed.

Zirconia Implants

Zirconia implants are a metal-free ceramic implant option. Many patients choose zirconia because they prefer a non-metal implant material or are seeking a more holistic or biologically oriented option. Zirconia is white in color, which can be advantageous in highly esthetic areas, especially for patients with thin gum tissue where a gray metallic hue may be a concern.

Zirconia implants are often described as a metal-free biologic implant option because they are ceramic, highly biocompatible, and designed for patients who do not want metal introduced into the body. They may be especially appealing for patients with metal sensitivities, esthetic concerns, or a strong preference for ceramic materials.

Modern zirconia implant studies are promising, with many reports showing survival rates in the mid-90% range in carefully selected cases, although the body of long-term research is still smaller than it is for titanium. Recent reviews report zirconia implant survival and success outcomes ranging widely depending on implant design, surface, surgical protocol, and follow-up period, with newer systems showing improved results compared with earlier generations.

Dr. Sarmiento’s Role in Implant Innovation

Dr. Hector Sarmiento’s expertise in implant dentistry extends beyond clinical treatment. He currently serves on the board of Advisors of a major implant company in Switzerland, where he contributes to the development of newer-generation titanium and zirconia implant technologies for future clinical use.

Which Implant Is Better?

There is no single implant material that is best for every patient. Titanium has the longest clinical history and the most extensive long-term data. Zirconia offers an excellent metal-free alternative with strong esthetic and biologic advantages in properly selected cases.
